Patients must know their vision plan name and eligibility prior to their routine eye exam.
Patients who are not eligible for eye exam coverage by their insurance company must pay for their exams out of pocket in full at the time of their appointments.
(We do not accept Aetna Better Health NJ, or Aetna Vision / Eyemed) Patients will need to pay out of pocket and submit to EyeMed for reimbursement.

We only accept AmeriHealth NJ if the patient has vision coverage under Davis Vision.
Patients for vision services must contact Blue Cross Blue Shield prior to their appointments to verify eligibility for eye exam coverage. At the end of the phone call, the insurance company will provide the patient with a reference number regarding the specific call.
Please provide that reference number to our staff at the time of your appointment.
Failure to do so will result in patients paying in full at the time of service and receiving an itemized receipt to submit to their insurance company for reimbursement.
(We do not accept Horizon NJ Health, NJ Family Care, or Anthem/Empire BCBS Vision AKA Blue View Vision.)
Patients who have not met their annual deductible must pay the full price of the exam, which will then go towards their deductible.
